Certain team activities may require that you provide Proof of Insurance in order to hold the event. Most common requirement for this Certificate is in the rental of ice outside of the PHA.

If you are required to provide a Certificate of Insurance then you should download the guide and forms and follow the directions provided to your team by the VP - OMHA Rep.
